Insurance Parental Waiver & Consent

I, the parent or legal guardian of the child listed below, hereby give my consent for my child to participate in the activities organized by Buddy Ball of Edison, Inc. I understand that participation in sports activities involves the risk of injury, and I assume all such risks on behalf of my child. I hereby waive, release, and discharge Buddy Ball of Edison, Inc., its officers, directors, coaches, volunteers, and representatives from any and all liability for injuries, damages, or losses that may occur as a result of my child’s participation.

 

I acknowledge that I have received and reviewed the concussion fact sheet provided by Buddy Ball of Edison, Inc. I understand that if my child is suspected of having a concussion, they may not return to play until they are evaluated and cleared by a licensed healthcare professional.
